Qualified Social Worker & x2013 Adults with Learning Disabilities

A Qualified Social Worker is required to join an Adults with Learning Disabilities team in South Yorkshire.

Working within the Adults with Learning Disabilities team, this Qualified Social Worker will have the following responsibilities:

- Provide a responsive person-centred social work service within a multi-disciplinary team.
- Developing care plans and managing caseloads.
- Monitoring and reviewing packages of support and maintaining up to date files.
- Use appropriate methods of assessment and intervention within relevant statutory and departmental requirements. This will include day services and supported accommodation provision.

The successful Qualified Social Worker will be required to carry out a specific piece of work, so experience within learning disabilities, carrying out assessments and reassessments would be advantageous.
