We are a local Provider of Domiciliary Care/Home care throughout the Wakefield Area. We are currently seeking to appoint a Care Co-ordinator for our Domiciliary/Homecare branch in Pontefract, West Yorkshire.

You will already have experience as Care Co-ordinator and as a Care Co-ordinator within our Domiciliary Care/Home Care Branch in Pontefract your duties will include

Care Co-ordinating the home care service involving the direct line management for a team of home care workers.

The Care Co-ordinator will participate as required in the assessment of people who use our home care service and the planning of the home care service users care.

To liaise with external professionals.

To ensure that the day to day home care service is operated in accordance with the National Minimum standards for Domiciliary Care.

It is essential that you will have held a Care Co-ordinator role in the past and are fully experienced in the provision of Domiciliary/home care or we would welcome applicants with a Care and office related background.
